Loose bulb, corroded bulb socket, or flakey wiring. I'd start by checking the bulb and bulb socket. You have to pull the hatch interior trim panel:

The bulb is just a standard incandescent bulb, a W5W, even though it sorta looks like it's LED. (probably designed to look like that to match the taillights that are on the body of the van, which are actually LEDs)
